A technician was tightening the hinges of a bathroom glass partition door as part of a minor maintenance request from the client. While performing the task, the glass door suddenly shattered, and multiple glass pieces fell, striking the technician on the head. He was not wearing a safety helmet, and as a result, sustained a head injury with minor bleeding. The technician was working alone, and the activity was not covered under a formal permit-to-work or risk assessment process. Initial Observations: The entire glass panel was shattered, scattered across the floor. The glass appeared to have pre-existing cracks and stress marks, indicating wear and tear. The technician was not wearing head protection (helmet) at the time of the incident. No second person or supervisor was present — lone working observed. No Standard Operating Procedure (SOP) or Hazard Identification and Risk Assessment (HIRA) for glass maintenance work was available. The PPE store had limited stock, and no record of PPE issuance was maintained. The task was handled as an immediate client request, without planning or equipment inspection. See more
