1) Mini-Flap - The periodontal tissue around a single tooth can be managed by mini-flap. It is a conservative flap design, with intra-sulcular incisions around the root caries involved tooth. A full thickness flap is then reflected for access to the carious lesion, without any vertical releasing incisions. 2) Conventional Flap - Access to a distal tooth in the arch, or difficult to access sites may require elevation of a wider flap involving more than one tooth. Normally extending the flap to one tooth beyond the carious tooth on either side will provide optimal access, however flap maybe extended to involve more teeth. A vertical release incision may provide additional access if necessary. Vertical incision in the lingual/palatal areas are avoided. These incisions must be placed at line angles of adjacent tooth to either include the interdental papillae or avoid it completely and must be long with a broad-based flap elevation to preserve blood supply to the flap.14 See more
