Reimagining Healthcare for Singapore's Longevity Dividend: A Framework for Managing Extended Lifespans in a Declining Birth Rate Society Abstract Singapore's Healthier SG programme represents a paradigm shift towards preventive, population-based healthcare. However, the programme's potential success in extending healthy lifespans to 90+ years for 60% of the population creates unprecedented policy challenges. This paper examines the systemic implications of healthcare success and proposes a comprehensive policy framework to manage extended lifespans amid declining fertility rates. We analyse the transformation required across healthcare financing, workforce planning, social infrastructure, and intergenerational policy design. The proposed framework addresses the fundamental question: how can Singapore maintain social and economic sustainability when healthcare success fundamentally alters demographic assumptions about aging, work, and care? Our analysis suggests that managing longevity dividends requires reimagining the social contract itself, moving beyond healthcare policy to integrated population management across multiple government domains.
